Hallucination rate is the evaluation metric that measures how often a model produces fabricated or incorrect information. TruthfulQA (2021), HaluEval, and Vectara's Hallucination Leaderboard are common references. Measurement is hard: human raters, LLM-as-Judge, or string-match against a control set are all used and each carries its own biases. In RAG systems with Grounding, hallucination rate turns into an operational KPI alongside Citation verification, and starts appearing in production SLAs.
